In the era of immunotherapy, image response processing is very challenging, whether its with CT, MRI or PET CT. Its very difficult to differentiate progression from response and from real progression. In the end, there is a wide variety of imaging findings with huge overlap over responders and non-responders. This is by far the biggest challenge in imaging immunotherapy for lung cancer.
