Integrated healthcare is vital to provide the optimal care to cancer patients at all stages, from diagnosis to remission and beyond. In this interview, Trisha Hatt from Macmillan Cancer Support, Glasgow, UK, discusses the Improving the Cancer Journey service in Glasgow, which takes a multi-partnership approach to work across the boundaries of acute, community and primary care. Ms Hatt provides an update on the outcomes and highlights the importance of pharmacists in this care model.
